Can Babies with HIE Be "Normal"?

Yes, some babies born with HIE (the abbreviation for the medical diagnosis of hypoxic ischemic encephalopathy) will fully recover over time and lead a normal life. In the case of extremely mild HIE, about 95% of the babies will develop no long-term complications.

But babies with moderate to severe forms of HIE brain injury may eventually suffer from cerebral palsy, delayed developmental milestones, vision, hearing, speech or cognitive impairments, intellectual disabilities, and/or seizure disorders. Many of these complications will not be known until baby is over the age of 2-3 years old.

In most cases, hypoxic ischemic encephalopathy (HIE) occurs around the time of labor or delivery. Once HIE brain injury is diagnosed, the medical provider will grade it as mild, moderate, or severe. In case of mild HIE, the medical team will closely monitor the baby to ensure they remain stable and do not require further treatment. In case of moderate to severe HIE, the medical team will perform a periodic developmental evaluation to assess motor or movement delays and diagnose cerebral palsy or other complications.

Parents should know that many cases of HIE called “mild” by their pediatricians and other doctors are actually more severe cases. Understating the significance of an HIE injury is a common error of doctors treating children suffering from HIE complications.

Estimates or Expectations of Normalcy after HIE

Each child’s condition will vary depending on the extent and severity of HIE birth injury they suffered. In general, the prognosis or expectations for a normal baby after HIE brain injury may be considered as follows:

  • Most babies with extremely mild HIE will survive and may not develop long-lasting or severe effects. The key question is what really is “mild” HIE injury. This may not be known for several years after the baby is born.
  • For babies with moderate HIE, about 30 to 50% may develop serious conditions, such as cerebral palsy, while about 10 to 20% may develop neurological problems.
  • Pertaining to babies that survive after severe HIE, at least 75% are likely to develop serious complications, such as cerebral palsy. The early mortality rate in case of severe HIE babies may be as high as 25 to 50%.
  • About 16% of the babies with moderate to severe HIE will develop seizure disorder or epilepsy.
  • About 14 to 17% of babies with moderate to severe HIE will develop vision impairment, while about 6% will develop hearing impairments.
  • Recent studies have shown that cognitive impairment in babies with HIE (many of these were initially called “mild”) may occur independently, even in the absence of cerebral palsy.

Increasing the Chances of Normalcy after HIE

Eligible babies with moderate to severe HIE injury should be administered hypothermia therapy within six hours after birth to reduce their risk of severe brain damage. MRI imaging tests should be performed early to determine a more accurate HIE diagnosis and plan for further assessments and treatment.

During the first two years, medical providers should closely monitor the development and progress of all babies affected by HIE. Professional development evaluations will focus on the baby’s motor, sensory, language, and cognitive (thinking) abilities in order to diagnose the degree of HIE and prepare an appropriate plan for therapy and support for the child and the family.
